Nexlizet (bempedoic acid/ezetimibe) can interact with other medications, including certain statins and corticosteroids.
Nexlizet is used in adults to reduce:
- low-density lipoprotein (LDL) cholesterol, along with a low fat diet and exercise and with or without a statin drug
- the risk of heart attack and coronary revascularization (a type of heart procedure) in people who cannot take a statin drug and have cardiovascular disease (CVD) or are at high risk of a CVD event
An interaction can occur because one substance causes another substance to have a different effect than expected. Interactions also can occur if you have certain health conditions.
Keep reading to learn about Nexlizet’s possible interactions. For a detailed overview of this drug, check out this in-depth Nexlizet article.
Before you start taking Nexlizet, tell your doctor and pharmacist about any prescription, over-the-counter, or other drugs you take. Sharing this information with them may help prevent possible interactions. (To learn whether Nexlizet interacts with herbs, vitamins, or supplements, see the “Are there other interactions with Nexlizet?” section.)
If you have questions about drug interactions that may affect you, talk with your doctor or pharmacist.
The table below lists drugs that may interact with Nexlizet. Keep in mind that this table does not include all drugs that may interact with Nexlizet. For more information about some of these interactions, see the “Drug interactions explained” section.

cyclosporine (Neoral, others) | — | can increase the risk of side effects from Nexlizet and cyclosporine |
Certain health conditions or other factors could raise your risk of harm if you take Nexlizet. In such cases, your doctor may not prescribe Nexlizet for you. These are known as contraindications. A contraindication of Nexlizet is:
- If you’ve had an allergic reaction: If you’ve ever had an allergic reaction to Nexlizet or any of its ingredients, your doctor likely won’t prescribe Nexlizet for you. This is because taking the drug could cause another allergic reaction. You can ask your doctor about other treatments that may be better options for you.
Before you start taking Nexlizet, talk with your doctor if the factor above applies to you. Your doctor can determine whether Nexlizet is safe for you to take.
Nexlizet is not known to interact with alcohol. But Nexlizet and alcohol can increase the risk of gout and elevated liver enzymes. So you may have an increased risk of these conditions if you drink alcohol during your Nexlizet treatment.
If you have questions about consuming alcohol while taking Nexlizet, talk with your doctor or pharmacist.

Interaction with statins
Nexlizet can interact with statin drugs, which are used to treat high cholesterol.
Examples of statin medications that may interact with Nexlizet include:
- simvastatin (Zocor)
- pravastatin (Pravachol)
What could happen
Taking certain statins during treatment with Nexlizet can raise your risk of side effects from the statins. These side effects include:
- muscle pain
- muscle weakness
- rhabdomyolysis, a severe condition in which damaged muscle breaks down
What you can do
If you’re taking a statin, tell your doctor before you start Nexlizet. They’ll likely decrease the dosage of your statin. You should not take a higher dose of either medication than your doctor prescribes.
If you take statins during Nexlizet treatment, tell your doctor right away if you have symptoms of muscle problems, such as:
- muscle weakness
- muscle cramping
- muscle pain
If you have questions about taking Nexlizet with statins, talk with your doctor or pharmacist.
Interaction with fibrates
Nexlizet can interact with fibrates, a group of drugs used to lower cholesterol. Some examples of fibrate medications are:
- gemfibrozil (Lopid)
- fenofibrate (Antara, others)
- clofibrate (Atromid-S)
What could happen
Taking fibrates during Nexlizet treatment can increase the risk of gallstones.
What you can do
Before you start Nexlizet treatment, tell your doctor if you take any fibrate medications. They can discuss whether Nexlizet is right for you or provide other treatment options.
Symptoms of gallstones include:
- pain in your back
- pain in your shoulder
- pain on the right side of your body
If you’re taking a fibrate drug with Nexlizet and you develop any of these symptoms, call your doctor right away.
Interaction with corticosteroids
Nexlizet also can interact with corticosteroids. These drugs relieve swelling and irritation. Some examples of corticosteroid medications are:
- prednisone (Deltasone, others)
- methylprednisolone (Solu-Medrol, others)
- hydrocortisone (Cortef, others)
- mometasone (Nasonex)
- prednisolone (Omnipred, others)
What could happen
Taking Nexlizet with corticosteroids can increase your risk of tendon rupture (a torn tendon). Tendons connect muscle with bone or some other structure.
What you can do
Before you start any corticosteroids, tell your doctor that you’re taking Nexlizet. They can determine whether it’s safe for you to take these drugs together. If so, they’ll likely monitor you closely for tendon problems.
Watch for symptoms of problems in tendon areas, such as:
- pain
- swelling
- tearing
- inflammation (usually indicated by redness or pain)
If you experience any of these symptoms in an area where a tendon might be, talk with your doctor right away. If your symptoms are severe or you feel or hear a snap or pop in the area, stop taking Nexlizet and get medical help right away.
